High Waist or Mid Rise? How to Choose the Right Incontinence Briefs

Both hold the same absorbent core. The difference is where the waistband sits — and that turns out to affect comfort, visibility under clothes and how securely they stay put.

It is a small decision that people spend a surprising amount of time on — and reasonably so. Underwear you are going to wear every day should feel right, and a cut that irritates you is a cut you will stop wearing.

Here is how to decide without guessing.

The 30-second answer

Choose high waist if

You want it to stay put

  • You need 350 ml capacity
  • You are active — walking, yoga, gym
  • You want support across the abdomen
  • You wear fitted trousers or dresses
  • You dislike a waistband cutting across the hip
Choose mid rise if

You want it to feel familiar

  • 90 ml is enough for your needs
  • You dislike fabric over the stomach
  • You wear lower-rise jeans or trousers
  • You run warm and want less coverage
  • You want it to feel like your usual underwear

The single most decisive factor: if you need more than 90 ml of absorbency, the decision is already made. Elva High 350 is only produced as a high waist — there is no 350 ml mid rise.

If 90 ml covers your needs, then it genuinely is a comfort and style preference, and both work equally well.

What actually differs between the two

Less than most people assume. It is worth being clear about this, because a lot of buying anxiety comes from imagining that one cut is somehow more protective than the other.

What is identical

The absorbent core sits in the gusset, not in the waistband. So an Elva MidRise 90 and an Elva High 90 hold exactly the same amount — up to 90 ml, through the same six-layer construction.

Both are registered as medical devices under EU regulation MDR 2017/745, both are OEKO-TEX® 100 certified, both are washable and reusable, and both use the same soft, breathable fabric blend.

What is different

Where the waistband sits, and what that does in practice.

High waist sits above the natural waistline, covering the lower abdomen. The band rests on the narrowest part of your torso, which is why it tends to stay in place.

Mid rise sits at a conventional height, roughly where most everyday underwear sits — below the navel, across the hip.

That is the whole difference. But it produces genuinely different daily experiences.

The absorbency question that settles it for many

Before considering style at all, work out how much capacity you actually need. Getting this wrong is the most common mistake — and it is usually in the direction of choosing too little.

Absorbency by cut — the women's Elva range
Product Cut Capacity Typical use
Elva MidRise 90 Mid rise 90 ml Light leakage, everyday wear
Elva High 90 High waist 90 ml Light leakage, everyday wear, exercise
Elva High 350 High waist 350 ml Heavier leakage, urgency, travel, overnight

So the logic runs in this order:

  1. Do you need more than 90 ml? If yes, you are choosing high waist — that is the only cut it comes in.
  2. If 90 ml is enough, then choose the cut you will actually enjoy wearing. Both give identical protection.
When in doubt, size up on capacity

Women consistently underestimate what they need — often because leakage varies from day to day, and a bad day is worse than the average. If you are hovering between the two levels, the security of extra capacity is worth more than the marginal difference in feel. You cannot over-absorb.

Five reasons to choose high waist

1. It stays where you put it

The higher band anchors against the narrowest part of your torso rather than sitting on the curve of the hip. That means less rolling, less shifting and less discreet readjusting during the day.

This is the most frequently cited reason women switch to high waist and stay with it.

2. It is better for movement

Bending, stretching, lifting and impact all pull at a lower waistband. A high waist band resists that.

If you do yoga, Pilates, walking or gym work, this is the practical difference between thinking about your underwear during a class and not thinking about it at all.

3. It is often less visible under clothes

Counter-intuitive, but true. Fitted trousers and skirts are tightest across the hip — exactly where a mid rise waistband sits. High waist places the seam above that pressure point, so visible lines are less likely.

4. Light support across the abdomen

The band provides gentle, even coverage over the lower stomach. Many women find this comfortable and reassuring, particularly during menopause when abdominal changes are common.

It is not shapewear and it does not compress. It simply sits smoothly.

5. It suits high-waisted clothing

If your wardrobe runs to high-waisted jeans, tailored trousers or dresses, a matching high rise avoids the awkward overlap of two waistbands at different heights.

Four reasons to choose mid rise

1. It feels like your normal underwear

For many women this matters more than any technical consideration. If you have worn mid rise your whole life, high waist can feel unfamiliar — and unfamiliar is a reason people stop wearing something.

The best incontinence underwear is the pair you actually put on.

2. Nothing over the stomach

Some women dislike fabric across the abdomen — after abdominal surgery, with a sensitive stomach, or simply as a preference. That is a completely legitimate reason to choose mid rise, and it does not cost you any protection at 90 ml.

3. Cooler in warm weather

Less fabric means less coverage and slightly better airflow. If you run warm, or you are managing hot flushes, mid rise can be the more comfortable choice through summer.

4. It works with lower-rise clothing

With mid-rise or low-rise jeans, a high waistband can sit above the trouser line. Mid rise avoids that entirely.

Worth saying plainly

High waist is not "the better product." It is one of two cuts of the same product. Some of our customers try high waist, find it sits higher than they like, and are happier in a lower cut — and that is a perfectly good outcome, not a compromise.

Matching the cut to what you wear

Quick reference by outfit
What you are wearing Better choice Why
High-waisted jeans or trousers High waist Waistbands align rather than overlap
Low or mid-rise jeans Mid rise Nothing sits above the trouser line
Fitted dress or skirt High waist No seam across the hip, smoother line
Leggings or gym wear High waist Stays anchored during movement
Loose, casual, everyday Either Pure preference — nothing to work around
Travelling or a long day out High waist 350 Capacity matters more than cut here

Getting the size right

Cut and size are separate decisions, and size is the one more likely to cause problems.

Too small and the waistband digs in, the gusset is pulled out of position, and the seal against the body is compromised — which is where leaks actually come from.

Too large and the underwear shifts, the absorbent core is no longer sitting where it needs to be, and the fit feels loose all day.

Measure rather than assume. Incontinence underwear is engineered to sit in a particular relationship to the body, and your usual high-street size is not always the right one. Our size guide has the measurements for each style.

If you order and the fit is not right, we exchange sizes free of charge. Nobody gets this right by guessing every time.

Why many women end up owning both

This is worth mentioning, because it is what a lot of long-term customers settle into after the first few months.

The two cuts suit different days rather than different people.

  • High waist for exercise days, fitted clothing, travel, long working days, and any day where staying secure matters most
  • Mid rise for warm weather, relaxed days at home, and lower-rise clothing
  • Elva High 350 kept for the days you know will be demanding, or for overnight

Since you need several pairs in rotation anyway — most women find three to five works well, allowing for washing — there is no real cost to that variety. You are buying the same number of pairs either way.

Common questions

Does high waist absorb more than mid rise?

No. The absorbent core is in the gusset, so Elva High 90 and Elva MidRise 90 hold identical amounts. The only reason high waist is associated with more capacity is that the 350 ml option is made exclusively in that cut.

Will high waist feel restrictive?

It should not. The band is designed to sit smoothly rather than compress — this is underwear, not shapewear. If it feels tight, the size is wrong rather than the cut.

Which is more discreet under clothing?

High waist, in most fitted clothing, because the seam sits above the hip where trousers and skirts pull tightest. Under loose clothing there is no meaningful difference.

I have had abdominal surgery. Which should I choose?

Many women who have had abdominal surgery prefer mid rise, since it avoids any fabric across the scar area. If you need 350 ml capacity and find the high waist uncomfortable over the abdomen, speak to your doctor or physiotherapist about what will work for your situation.

How many pairs do I need?

Three to five suits most women — one being worn, one in the wash, and spares. They wash at 60 degrees with your normal laundry.

Can I switch between the two?

Yes, and many women do. There is no adjustment period and no reason to stay loyal to one cut.

In short

If you need more than 90 ml, choose high waist — that is where the capacity is.

If 90 ml is enough, choose the cut you will look forward to putting on. High waist for security and movement; mid rise for familiarity and lightness. The protection is identical, and there is no wrong answer.
